Student: They are: assessment of patient; planning of care; implementation of care and evaluation of care given.

T: What do you do when you assess the patient?

老師:你在估計(jì)階段都做什么?

S: We observe the patient using the senses such as looking, listening, touching, smelling an speaking. An active assessment of the patient's needs.

學(xué)生:運(yùn)用感覺(jué)器官觀察病人,如:望、聽(tīng)、觸、嗅和語(yǔ)言,要對(duì)病人的需要做一個(gè)切實(shí)的估計(jì)。

T: What is nursing diagnosis?

老師:什么是護(hù)理診斷?

S: Nurasing diagnosis is a statement of an actual or potential patient problem that requires nursing intervention.

學(xué)生:護(hù)理診斷是確定病人現(xiàn)存或潛在的需要采用護(hù)理措施的問(wèn)題。

T: How do you make the plan of care?

老師:怎樣制訂護(hù)理計(jì)劃?

S: The care plan is built on the foundation of patient's problems. The highst priority will be designed as problem number one and treated first. For example the oxgen need of a patient with myocardial infarction must always be met at once.

學(xué)生:我們以病人的問(wèn)題為根據(jù)制訂護(hù)理計(jì)劃。確定急需解決的問(wèn)題,要首先予以解決。例如心肌梗塞病人的氧氣需要總是必須立即滿足的。

T: What is the meaning of implementation?

老師:實(shí)施計(jì)劃是什么?

S: That is to carry out the plan.
